Supported Independent Living

We are a service provider of Supported Independent Living (SIL) which is a type of support provided for people with a disability who have higher support needs and predominantly require assistance at home all the time.

We facilitate access to shared or group home accommodation living arrangements for our participants.

Each of our participants will need to have a plan in place that stipulates their individual:

Respite Care

Primary carers of a person with a disability need to take regular breaks for their mental and physical wellbeing.

we provide respite care; taking on all the daily tasks and activities that are normally undertaken by a primary carer to ensure the participant receives the same level of care they are used to

Assistance with daily task

We can help you with everyday tasks: including 

  • personal care,
  • meal preparation,
  • house cleaning,
  • garden maintenance,
  • banking tasks and attending appointments.

HOW SUPPORT COORDINATION WORKS

It’s different for every person and there are a hundreds of services providers available.

We work with you to find,
understand and access the ones that are right for you.

Step 1. We can Meet with you in a place that’s comfortable e.g your home, school or even a café

Step 2. Understand your needs, goals and what matters to you

Step 3. Write a support plan with clear actions to reach your goals

Step 4. Connect you with local support e.g speech therapist, cooking class, trainer etc

Step 5. Work with you to track your progress and adjust support to your needs

Step 6. Help build your capacity to manage your own supports independently over time

Support coordination is all about maximising your NDIS plan so you can feel constantly comfortable, supported and ready to handle life.
